Form - pdf format This form should be completed, printed, and turned into the meet entry folder at the high school pool 2010 USA Athlete Registration Form - pdf format This form needs to be completed by December 31, 2009. Completed forms need to be deposited in the marked envelope at the Laramie High School pool. Do not put the forms in the payment boxes at either the high school or rec center. This form is for coaches and officials. LSC Liability Release Form - pdf format This form needs to be completed by new members, and by existing members every fall. The form can be sent to the P.O. box or dropped in one of the locked boxes at the HS or Rec Center. Consent for Emergency Care Form - pdf format This form needs to be completed by new members, and by existing members every fall. The form can be sent to the P.O. box or dropped in one of the locked boxes at the HS or Rec Center. E-mail Request Form - pdf format This form needs to be completed by new members and any existing members not on the e-mail list. The form can be sent to the P.O. box or dropped in one of the locked boxes at the HS or Rec Center. LSC Volunteer Form - pdf format This form needs to be completed by new members and any existing members who have not indicated their volunteer preferences.
